Long term care provides professional nursing services around the clock to people who are no longer able to stay at home safely
Provides long term care for adults who can no longer live safely or independently at home due to their need for 24-hour nursing supervision in a secure environment, and their unpredictable or complex care needs which can no longer be managed in their home setting.
Additional services include personal care assistance, end-of-life care, meals, nutritional support, social work, and recreational activities. Do not contact the care home directly to reserve a space as access to long term care is made through assigned case managers who first determine eligibility.

Eligibility
Clients must meet Home and Community Care requirements and criteria for admission. A health care needs assessment will be completed to confirm eligibility. See the eligibility requirements for home and community care section of our website.
Referral
Referral by case manager at a Home and Community Care office is required. To access service call the Home Health Service Line for more information; do not contact the care home directly.
Fees
There is a monthly accommodation fee; additional fees may apply for incidental services.
